KVO Client Patient Forms
Please click on the PDF link above and print the attached form, which contains client/patient information and ophthalmology history. Please bring the completed, signed forms to your appointment.

Preparing for Your Visit

Referrals from primary care veterinarians are welcome, but not required in order to schedule an appointment.

Whether you are visiting us for the first time or returning for a recheck, please always bring all of the medications currently being used to treat your pet’s eyes (topical and oral) with you to every appointment.

If possible, have the new client/patient paperwork (3 pages) completed if you are coming in for your first visit.

If you need to cancel or reschedule an appointment, please provide at least 48 hours notice. Less than 48 hours notice will be considered a no show, and after two such instances pre-payment may be required to reserve your appointment.
